The icons used on the screenshot are NuoveXT, I think, but there are also some custom icons pulled from different themes (Firefox looks like its take from Polar theme or something like that, trashcan seems to be from Mist, but I'm not sure).
Don't know about the GTK theme, I'm not particularly fond of Vista-esque themes myself
The window manager is either Emerald (from Beryl) or Compiz - transparencies are obviously enabled.
Look through gnome-look.org and see what you can find.
